Connected Care Programme – Impact Hub

At These Hands Academy, the Connected Care Programme works alongside the Impact Hub by turning learning into consistent everyday practice. While the Impact Hub gives staff the knowledge and confidence to recognise deterioration, take observations and communicate concerns using tools such as Soft Signs, NEWS2 and SBAR, the Connected Care Programme supports organisations to embed those skills through telehealth, structured onboarding, data review and continuous quality improvement.
