AIMS and OBJECTIVES:
  • Why Person centred planning is central to good care practice.
  • Gain knowledge around a person centred approach to support planning.
  • Understand how person centred planning relates to the Care act.
  • Be familiar with and able to use a range of Person centred planning tools.
  • To look at planning creatively to enable service users to identify their individual outcomes and to produce a plan which will meet those outcomes.
  • This session maps to standard 5 of the care certificate “Working in a person centred way
TARGET GROUP: Care home managers, staff who have contact with people with a learning disability. Dates and venues available

Times: Arrive 9.15 for registration and refreshments for a 9.30am start and finish 4.30pm Refreshments will be provided.   Lunch will NOT be provided For this course care providers will be invoiced at a flat rate of £30 per delegate, after the attendance. If delegates fail to attend, the establishment will be invoiced for the non-attendance rate of £100 per person.
